According to what Osamu Nishimura told Nikkan Sports, a couple of weeks before Karl Gotch's death, he had undergone emergency surgery to remove an aortic aneurysm, and never fully recovered.

I'm no rocket surgeon, but in layman's terms, Gotch had a bubble in aorta, which is, like, some kind of important vein.
